         PLEASE NOTE:that Conference dates
         on registration form have been changed,
         the correct conference dates are
         October 20-22.

         Registration fees do not include hotel
         accomodations or transportation costs.
         Hotel reservations must be made directly
         with the hotel. Registration received after
         October 10, 2004 will be considered on-site
         ($275.00 fee will apply.)
         Written cancellation received before
         October 10, 2004 will receive a full refund
         less $50 processing fee. Cancellation
         after October 10, 2004 are non-refundable
         Registration includes admission to all
         presentations, conference packets,
         continental breakfast, lunch, refreshment
         breaks, and barbecue reception following
         the final presentation.

         *8% tax not included.
         Specific accomodation requests granted pursuant to availability.
         $10 per day service charge.
         In order to receive the conference rate
         of $106 per night, you must mention that
         you will be attending the VIRGIN ISLANDS

         Reservations must be made directly with
         the hotel prior to September 20, 2004
         and require a credit card or money order
         for a one night's deposit. Specific requests
         may be made directly with the hotel
         reservations department. Cancellations
         received after September 20, 2004 and
         no-show will be billed for one night's
         room and tax.
