         CONFERENCE REGISTRATION
         Registration fees do not include hotel
         accomodations or transportation costs.
         Hotel reservations must be made directly
         with the hotel. Registration received after
         October 10, 2004 will be considered on-site
         ($275.00 fee will apply.)
         
         
         CANCELLATION POLICY
         Written cancellation received before
         October 10, 2004 will receive a full refund
         less $50 processing fee. Cancellation
         after October 10, 2004 are non-refundable

         Registration includes admission to all
         presentations, conference packets,
         continental breakfast, lunch, refreshment
         breaks, and barbecue reception following

         Reservations must be made directly with

         and require a credit card or money order
         for a one night's deposit. Specific requests
         may be made directly with the hotel
         reservations department. Cancellations
         received after September 20, 2004 and
         no-show will be billed for one night's
